Apply Today Schedule: Monday–Friday | 9:00 AM–5:00 PM Location: Denver, Colorado, 80221 Job Type: Local contract Contract Length: 6 Weeks Contract (Possibility to Extend) Pay Rate: $54.00/hour (W-2) Plus Mileage reimbursement *Pay offered is based on experience, expertise, credentialing, and education.
Duties: Assess home health and/or hospice patients in both stable and emergent situations to identify physical, psychosocial, and environmental needs. Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. Obtain and incorporate physical, psychological, social, spiritual, and cultural information that may influence the patient's health status. Maintain accurate and current patient records, documenting all visits, contacts, problems, plans, interventions, goals, and changes in condition. Provide patient care utilizing appropriate infection-control measures in accordance with OSHA standards. Educate patients and families regarding medications, diet, treatment plans, home safety, disease management, and other aspects of care.
Requirements: Education: Graduate of an accredited nursing program License(s): Registered Nurse (RN)
