The Physical Therapist provides one-to-one care in patients homes, with responsibility for visits, clinical documentation, and timely communication. The role involves up to six patient visits per day and requires skilled home health experience.

Job Details Schedule: 8-hour day shifts, 40 hours per week; Monday orientation. Weekend coverage is not required.
