Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 11/23/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Summary Physical Therapist, Home Health Mt. Angel, OR and surrounding areas. This role provides one-to-one skilled home health physical therapy services across an assigned territory. The position includes daytime visits, electronic documentation, and care for multiple patients in a community-based setting.

Job Details Schedule: 8-hour day shifts, 40 hours per week, starting 11/23/2026.

Required Qualifications: Minimum of 1 year of skilled home health experience; Basic Cardiac Life Support (BCLS) certification; OASIS experience for clinicians completing Start of Care assessments; proficiency with the required home health charting system; active drivers license and personal transportation; primary COVID-19 vaccination series plus one booster; license in hand at submission.

Additional Information: One-to-one patient care with up to 6 patient visits per day. Standard productivity expectation of 30 points. Timely communication and documentation are required. Orientation is held on Mondays. Business casual attire is required; scrubs are permitted without a designated color. Home health physical therapy services are provided in Mt. Angel and surrounding communities.
