Details Home health setting covering assigned territory in Richmond and surrounding areas (Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington) plus either the Oakland/Berkeley area or the Vallejo/Napa area Day shift schedule with possible weekend coverage required May require travel outside the assigned service area to maintain a full caseload Caseload includes visits such as routine, evaluation, discharge, OASIS SoC/RoC/ReCert, and follow-up visits Willingness to see COVID-positive and COVID PUI patients may be required Requirements Current, active CA Physical Therapist license required Minimum 1 year of recent experience required BLS certification required (license required if applicable) Valid driver's license, auto insurance, and auto registration required Required immunizations/documents: annual TB test, MMR, Varicella, HepB, Tdap (within 10 years), annual flu vaccine, drug screen, and physical exam Additional Information Responsibilities include performing home health visits, evaluations, re-certifications, and documentation for an assigned patient caseload May require travel beyond the primary service area to meet visit expectations Former employees of this facility are not eligible for this assignment All requested time off must be disclosed prior to assignment confirmation; time off requested after acceptance will not be approved First-time travelers may be considered for this role American Traveler Job ID P-758458.
