The position evaluates patients and develops, implements, and modifies individualized treatment plans based on patient needs, goals, and physician direction. The role also provides patient education, coordinates discipline-specific therapy services, and supports patients functional return to community activities.

Job Details Required Qualifications: Degree in Physical Therapy from an accredited school; 1 year of outpatient clinical experience; current Cardiopulmonary Resuscitation (CPR) certification; active and applicable state license for the work location.

Schedule: Day shift; Monday, Wednesday, and Friday from 8:00 a.m. to 5:00 p.m.; Tuesday and Thursday from 9:00 a.m. to 6:00 p.m.; 40 hours per week.
