Provides services utilizing nursing theory and processes, communicates/documents observations and assessments, promotes and maintains health of individuals through direct care, teaching, counseling and appropriate rehabilitative measures. MINIMUM REQUIRED QUALIFICATIONS – MA RN license BLS 2 years recent home health case management experience LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – M-F 8am -4:30 pm with weekend rotation of 1 /4 weekend requirement START DATE – 11/22/2026 Facility Location Northampton is a beautiful town filled with historical buildings.
