Prime Staffing is seeking a travel nurse RN Case Manager, Acute Care Case Management for a travel nursing job in Eureka, California.

Job Description & Requirements Specialty: Acute Care Case Management Discipline: RN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel We are seeking an experienced Registered Nurse specializing in Case Management to coordinate patient care, facilitate safe and timely discharges, and collaborate with interdisciplinary healthcare teams in an acute care setting. The ideal candidate will have strong care coordination, discharge planning, patient assessment, and documentation skills.

Responsibilities: Coordinate patient care in collaboration with physicians, nursing staff, social workers, and other healthcare professionals. Develop and facilitate individualized discharge plans to support safe transitions of care. Assess patient needs, discharge barriers, and available healthcare resources. Coordinate post-acute services, follow-up care, and community resources. Assist with utilization review and communication with insurance providers as needed. Maintain accurate documentation using the Epic electronic health record system. Collaborate with patients and families to support continuity of care.

Requirements: Active Registered Nurse (RN) license appropriate for the assignment. Minimum 2 years of relevant Case Management experience. Acute care experience required. First-time travelers may be considered subject to Clinical approval. Ability to work every other weekend. Flexibility to float to similar units based on facility needs. Proficiency with Epic charting preferred/required per facility workflow.
