Will attend interdisciplinary rounds. Complete Initial assessments. Facilitates the development of a plan of care in collaboration with the patient, family, caregiver and health care team. Communicate consistently with medical teams and patient regarding progress of care needs related to post discharge care needs, such as: home health, inpatient rehab. Will interact with post acute care referral sources to establish a plan. Document and Place orders for home health, DME, nutritional needs within EPIC EMR. MINIMUM REQUIRED QUALIFICATIONS – NH/compact RN license BSN Certified Case Manager (CCM) 3 years inpatient clinical experience 1 year Care Management experience LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 8-4:40 M-F. No weekends or on call. SYSTEMS – Epic START DATE – 11/23/2026 Facility Location Lebanon is located in idyllic Grafton County, just south of Hanover.
