Cross Country Allied is seeking a travel Physical Therapist for a travel job in Hesperia, California.

Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 11/23/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Summary Physical Therapist, Home Health. The role provides in-home physical therapy services for adult and geriatric patients, with a focus on mobility, rehabilitation, and patient and family education. The assignment is scheduled to begin on 11/23/2026 and is expected to run through 02/22/2027.

Job Details Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.

Required Qualifications: Minimum of 1 year of recent relevant experience; Basic Life Support (BLS) certification; active license in hand at submission; active drivers license and personal transportation; timely communication and documentation; ability to provide care for geriatric patients; and proficiency with Homecare Homebase (HCHB).

Preferred Qualifications: Experience with electronic charting systems.

Additional Information: Provides care for adults and geriatric patients in a home health setting. Typical caseload includes approximately 57 visits per day or a minimum of 30 points per week. Patient care may include geriatric rehabilitation, post-surgical recovery, stroke rehabilitation, neurological conditions, spinal conditions, joint replacements, and patient and family education. Orientation is scheduled for Monday. On-call rotation may be required. Scrubs of any color are permitted; jeans are not permitted. A tablet is provided for clinical documentation. A home health checklist is required. Requested time off must be submitted at the time of application.
