Details Home health setting serving a geriatric patient population Monday through Friday schedule with flexibility to work weekends Average caseload of 5-7 patients per day 6 visits daily or a minimum of 30 points weekly No on-call or weekend requirements Territory covers up to a 60-mile radius from the hub office iPad provided for documentation Requirements Current OT license required (license required if applicable) Minimum 1 year of recent relevant experience OASIS experience required for completing Start of Care visits Proficiency in HCHB (Homecare Homebase) EMR required BLS certification required Active driver's license and personal transportation required License must be in-hand at the time of consideration Additional Information Timely communication and documentation are mandatory Hospice/Home Health checklist required for consideration Monday orientation required, including completion of billable modules Scrubs required, any color, no jeans permitted Mileage reimbursed at the IRS standard rate RTO requests must be submitted at the time of application On-call rotation may be required, to be discussed during interview First-time travelers will not be considered Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler Travel pairs accepted, though each traveler is assigned their own territory and must have personal transportation Local candidates accepted at the travel rate American Traveler Job ID P-772044.
