This role focuses on providing essential therapy services in a home health setting, ensuring patients receive the highest standard of care.

Shift Details Schedule: Monday - Friday Hours: 8:00 AM - 5:00 PM Weekend Work: No weekends required Responsibilities Conduct start of care assessments, recertifications, and resumption of care for patients. Manage a typical workload of 5-6 patient visits per day, varying based on admissions or assessments. Collaborate with clinical coordinators and directors for support in the field. May assist in safety evaluations or teaching visits for hospice patients, as required.

Qualifications Experience: Minimum of 1 year of experience, preferably in home health care with OASIS training.

Certifications: Current BLS certification and a valid state Physical Therapy License are required.

Additional Requirements: Current Driver's License and Auto Insurance are mandatory. Orientation New hires will participate in a short orientation, working alongside experienced staff for 1-2 weeks.
