This is a night shift role requiring 12-hour shifts with guaranteed hours of 36 per week. The role entails facilitating patient transfers, conducting assessments, and coordinating with medical professionals across various facilities. Location Rancho Cordova, CA Job Responsibilities Facilitate transfers for various patient types from hospitals within and outside the organization. Conduct patient assessments, including gathering vitals and clinical information post-intake process. Collaborate with accepting physicians and specialists to arrange for patient admissions. If working as an LVN, prioritize referral processes if no internal placements are available.

Qualifications Minimum of 3 years of nursing experience in ICU, Emergency Department, or transfer center. Acute care experience is essential. Critical care assessment skills and sound clinical judgment are required.

Ability to follow established protocols and algorithms. Professional demeanor with strong customer service skills is necessary. Proficiency in computer applications, including Cerner or EPIC, TeleTracking, and Google apps.

Requirements Current RN or LVN license. ACLS, BLS, and NIHSS certifications. Ability to work flexible shifts, including evenings, weekends, and holidays as needed.

Limited to no requests for time off (RTO). Orientation and Training Orientation includes one day of general hospital training followed by 3-5 days of department-specific training, which may occur across various shifts. Dress Code Scrubs of any color are permitted, provided they are clean and professional.

Casual attire such as jeans without rips or holes and clean hoodies is acceptable.
