Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 11/16/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Summary Physical Therapist, Physical Therapist Outpatient Southern Indiana. The position provides outpatient physical therapy services through a home health division, including patient evaluations, individualized treatment planning, and ongoing care modifications. The Physical Therapist supports functional recovery, patient education, professional development, and safe, high-quality care delivery.

Job Details Responsibilities: Evaluate assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, physician direction, and established length-of-stay expectations. Provide discipline-specific therapy services, support functional return to community activities, educate patients and families, document care accurately and promptly, participate in student development when assigned, maintain professional licensure, and contribute to quality improvement initiatives.

Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; current cardiopulmonary resuscitation (CPR) certification; professional licensure in good standing; and completion of applicable state-mandated continuing education requirements.

Schedule: Day shifts, 8 hours per shift, 40 hours per week.
