Job Summary: Seeking an experienced Home Health RN to provide skilled nursing care to geriatric patients in home settings, complete patient assessments, and maintain accurate clinical documentation.

Requirements: Active CA RN license or pending license application Minimum 1 year of Home Health experience with geriatric patients OASIS experience required BLS certification (AHA) Valid driver's license and reliable personal transportation Experience with WellSky and TigerText preferred Ability to complete 25 visits per week On-call rotation may be required Additional Details: Mileage Reimbursement: Mileage traveled between patient visits is reimbursed at the IRS standard mileage rate.
