This role provides skilled occupational therapy services in patients homes, with one-to-one care and up to six patient visits per day. The position offers weekday, eight-hour shifts beginning 11/23/2026 and requires skilled home health experience.

Job Details Schedule: Day shifts; 8-hour shifts; 40 hours per week.

Start date: 11/23/2026.

Required Qualifications: Oregon occupational therapy license; Basic Cardiac Life Support certification; minimum of 1 year of skilled home health experience; OASIS experience for Start of Care assignments; active drivers license; personal transportation; timely communication and documentation.

Preferred Qualifications: Previous travel experience and experience with home health electronic documentation platforms.

Additional Information: One-to-one patient care with up to six patient visits per day. Standard productivity expectation of 30 productivity points. Business-casual attire is required; scrubs are permitted without a specific color requirement. Orientation is scheduled for Mondays. On-call rotation may be required and will be discussed during the interview. Primary COVID-19 vaccination series and one booster are required.
