Develop personalized treatment plans focused on improving mobility, strength, balance, safety, and functional independence. Assist patients recovering from hospitalization, surgery, illness, injury, or changes in functional status. Provide therapeutic exercise, gait training, transfer training, balance activities, and fall-prevention interventions. Assess patients' home environments and recommend strategies to improve safety and independence. Educate patients and caregivers on home exercise programs, mobility techniques, and injury prevention. Monitor patient progress and adjust treatment plans to support functional goals. Collaborate with physicians, nurses, occupational therapists, and other home health professionals. Complete accurate and timely clinical documentation in accordance with home health requirements. Travel between patient homes within the assigned service area.

Requirements Active Oregon Physical Therapist license or eligibility to obtain. Previous Home Health experience preferred.

Experience with OASIS assessments and home health documentation preferred. Current BLS certification. Valid driver's license and reliable transportation.

Strong clinical assessment, communication, and patient education skills. Ability to work independently and manage a community-based patient caseload. Excellent time-management and organizational skills.
