Job Title Physical Therapist – Travel Location Kissimmee, FL | Home Health Schedule / Job Type Contract | Monday–Friday, 8:00 AM–5:00 PM | Onsite/Field-Based Key Responsibilities Provide skilled physical therapy services to home health patients across an assigned coverage area within Osceola County Conduct and document OASIS assessments as required for qualifying visits Manage a caseload averaging 3–4 visits per day when OASIS-type visits are included, or a higher volume of standard visits as assigned Travel within the assigned coverage area, averaging approximately 35 miles per day Comply with facility call-off policies, including potential weekend or holiday rescheduling if a call-off occurs on a weekend or holiday Required Qualifications Active Physical Therapist (PT) license in the state of Florida Experience with OASIS documentation in a home health setting Valid driver's license and reliable transportation for daily field travel Ability to manage an independent caseload with minimal supervision Preferred Experience / Skills Prior home health experience, particularly in a travel or contract capacity Familiarity with Osceola County geography and surrounding service areas Experience working alongside or supervising PTAs in a home health environment Why Join Us 13-week contract with an expected 40 hours per week, offering consistent, stable field-based work IRS mileage reimbursement provided to support daily travel requirements Concentric Healthcare Staffing Job ID 798861.
