The ideal candidate will provide exceptional rehabilitation services to patients, helping them recover and regain mobility.

Responsibilities Assess and evaluate patient conditions and needs for physical therapy interventions. Develop and implement individualized treatment plans based on patient evaluations. Provide direct patient care in accordance with established protocols and standards.

Collaborate with multidisciplinary teams to ensure comprehensive patient care. Maintain accurate documentation of patient progress and treatment plans. Educate patients and their families on therapeutic exercises and self-care techniques.

Participate in quality improvement initiatives and ensure compliance with regulatory standards.

Qualifications Degree in Physical Therapy from an accredited program. State licensure or eligibility for licensure as a Physical Therapist.

Experience in acute care hospital settings preferred. Proficiency in electronic health record (EHR) systems, specifically Epic. Strong communication and interpersonal skills.
