Current HCPC registration . Significant experience working as a Band 6 equivalent Occupational Therapist with children and young people with autism, severe learning difficulties and complex neurodevelopmental needs. Proven experience in the supervision, line management or clinical mentorship of junior staff or Therapy Assistants.

Job Summary: Nurse Case Manager will address the needs of students with chronic health conditions so students with these conditions may attend school safely. Nurse Case Manager will assist in initiating a “Bridge” re-entry program, with adjustment counselors and other school staff, for students returning to academics after extended absence for reasons such as post hospitalization, rehabilitation or recovery, or concussion, to successfully return to learning. Nursing Case Manager will assist in implementing additional projects such as responding to state and local emergencies, pandemic response, changes in state mandates and requirements for school health services, etc. Nurse Case Manager provides an annual community health needs assessment that addresses health inequities and the impact of institutional racism and socioeconomic disparities on students and their academic achievement.
