Serve as the central clinical coordinator for the multidisciplinary team, managing the implementation of care plans and providing real-time updates on patient condition. Facilitate a seamless transition through the healthcare system by assessing discharge needs and collaborating with the team to prepare patients for their next stage of recovery.
