Package Description: Use your clinical expertise to help patients get the right care, first time. Every day, thousands of people contact NHS 111 looking for urgent advice, reassurance and clinical support. As an IC24 Clinical Advisor, you'll be at the heart of those conversations. Using your professional judgement, advanced assessment skills and NHS Pathways, you'll assess patients over the phone, provide evidence-based clinical advice and ensure they receive the right care, from the right service, at the right time. No two calls are the same. One moment you could be supporting a worried parent whose child has developed a fever. The next, assessing a patient with chest pain, helping someone experiencing a mental health crisis or identifying safeguarding concerns that requires immediate action. Working as part of a multidisciplinary team, you'll combine your clinical expertise with excellent communication skills to make safe, timely decisions that have a direct impact on patient care and outcomes. This isn't traditional frontline nursing or paramedic practice. It's a role where your assessment skills, clinical judgement and ability to stay calm under pressure can make a difference to thousands of patients every week. Whether you're looking for greater flexibility, a new clinical challenge or a role that allows you to use your expertise in a different way, this is an opportunity to build a rewarding career in urgent care. To learn about a typical day in this role, watch our video here: What you'll be doing → Delivering expert clinical assessment Assess patients presenting with a wide range of physical health conditions, injuries, mental health concerns and safeguarding issues using NHS Pathways and your professional clinical judgement. → Providing safe clinical advice Deliver evidence-based self-care advice, safety-netting information and onward referrals, ensuring every patient receives the most appropriate outcome. → Supporting patients through difficult situations Manage complex, sensitive and emotionally challenging conversations with empathy, professionalism and reassurance. → Supporting colleagues Provide clinical guidance to Health Advisors during escalated or complex calls, helping ensure patients receive safe and effective care. → Making timely clinical decisions Assess risk, prioritise patient safety and make informed clinical decisions within your professional scope of practice. → Working collaboratively Work closely with colleagues across the wider urgent care system to support effective patient pathways and continuity of care. → Maintaining professional excellence Keep your clinical knowledge up to date through training, supervision, competency assessments and continuing professional development. → Promoting safe, high-quality care Recognise safeguarding concerns, contribute to service improvements and uphold IC24's values of Care, Excellence, Respect and Innovation in every interaction. Who this role suits This role is ideal for registered Adult Nurses and Paramedics who enjoy clinical assessment, critical thinking and making informed decisions in a fast-paced environment.
You'll thrive if you: Are a registered Adult Nurse or Paramedic with at least two years' recent post-registration experience in primary care, urgent care or emergency care. Have excellent assessment, communication and clinical decision-making skills. Remain calm and confident when managing complex or emotionally challenging situations. Enjoy using your knowledge to assess patients remotely and guide them towards the right care. Can balance empathy with sound clinical judgement. Work well independently while contributing to a supportive multidisciplinary team. Are confident using technology and multiple clinical systems. Are committed to continuous professional development and maintaining clinical excellence. Can work flexibly across a 24/7 rota, including evenings, weekends and bank holidays. Previous telephone triage or NHS Pathways experience is welcomed but not essential, as full training will be provided where required.
Why IC24?
Working in urgent care is demanding, but you'll never face those challenges alone. At IC24, you'll join an experienced multidisciplinary team committed to delivering outstanding patient care while supporting one another every day. You'll receive comprehensive induction, ongoing clinical supervision, regular coaching and continuous professional development to help you grow throughout your career.
Following six months in post, hybrid working may be considered, subject to individual performance and service requirements. As a leading not-for-profit social enterprise, everything we do is focused on improving patient care while creating an environment where our colleagues feel valued, supported and empowered to develop. Let's be honest about the role This role isn't for everyone.
You'll make important clinical decisions every shift, often supporting patients experiencing distressing symptoms, mental health crises or safeguarding concerns. The pace can be fast, and you'll need to adapt quickly as priorities change throughout the day. Working remotely means your assessment, communication and decision-making skills become even more important.
You'll need to gather detailed information, manage uncertainty and make safe clinical decisions without seeing the patient face-to-face. What can't be taught is clinical curiosity, emotional resilience, sound judgement and a genuine commitment to delivering safe, compassionate care. If those qualities describe you, we'll provide the training, support and development to help you succeed.

Package Description: Advanced Clinical Practitioner – Home Visiting Permanent | Up to 37.5 hours per week Bring advanced urgent care to patients who need to be seen at home. Not every patient who needs urgent care can come to us. Some are housebound, frail, approaching the end of life or have been clinically assessed as needing a face-to-face review in their own home. What they need is an experienced clinician who can assess the whole picture, make confident autonomous decisions and put the right care in place. At IC24, we're looking for an Advanced Clinical Practitioner to join our Home Visiting Service across Sussex. You will have access to IC24’s fleet of vehicles and any equipment required to carry out this role, we only need you to bring your clinical expertise. You'll work autonomously in patients' homes, using advanced assessment, examination, diagnostic and prescribing skills to provide safe, effective and patient-centred urgent care. You'll care for patients across the full General Practice population, including children, older people, frail patients and those with mental health or end-of-life needs. If you enjoy clinical autonomy, variety and the challenge of making safe decisions outside a traditional clinical environment, this could be the role for you. What you'll focus on → Assessing and treating patients in their own homes You'll provide face-to-face assessment, diagnosis and treatment for patients who have been clinically triaged as needing a home visit. You'll take a detailed history, carry out a physical examination and use advanced clinical reasoning to establish a differential diagnosis and agree the most appropriate treatment or care plan. You'll see a wide range of urgent and primary care presentations, often without immediate access to the investigations and resources available in a hospital or clinic. → Making confident autonomous clinical decisions Home visiting requires strong clinical judgement. You'll often be assessing patients you've never met before, sometimes with limited information and sometimes in complex or emotionally difficult circumstances. You'll need to be comfortable managing uncertainty, prioritising clinical risk and deciding whether someone can be safely treated and supported at home or needs escalation to another service. You'll always work within your own competence and know when to seek further clinical input. → Treating the whole patient Seeing someone in their own home gives you a different perspective on their health. You'll care for patients across the full spectrum of General Practice, including newborns and children, adults, older and frail patients, people with mental health needs and those approaching the end of their lives. You'll consider the patient's physical health alongside their environment, support network and wider social circumstances. That means looking beyond the immediate symptoms and understanding what the patient genuinely needs to remain safe. → Prescribing and managing treatment at home As an independent prescriber, you'll develop and deliver evidence-based treatment plans. You'll prescribe safely within agreed clinical guidance, review existing medication where appropriate and make sure patients and carers understand what happens next. Where required, you'll undertake point-of-care testing, arrange appropriate investigations and interpret available clinical information to support your decision-making. → Keeping patients safely in their community One of the most important parts of home visiting is helping patients receive the right care in the right place. Where clinically appropriate, you'll help patients remain safely at home by putting appropriate treatment, referrals, advice and safety-netting in place. You'll work with primary care, community nursing, pharmacy, social care, ambulance services and secondary care where needed to support continuity of care. And when hospital or another service is the right place for a patient, you'll recognise that and escalate appropriately. Whether you're treating someone at home, referring them to another service or escalating their care, the patient and their safety will be at the centre of your decision-making. → Supporting palliative and end-of-life patients Some of the most important visits you'll make will be to patients approaching the end of their lives. You'll assess symptoms, prescribe and manage treatment where appropriate and work sensitively with patients, relatives and carers during what can be a difficult and emotional time. You'll use your advanced clinical judgement alongside existing care plans and local guidance to help make sure patients receive compassionate, appropriate care. → Working independently, but never in isolation Home visiting is autonomous by nature. You'll spend a significant part of your working day out in the community, so you'll need to be confident managing your own workload and making decisions independently. But you'll remain part of a wider IC24 clinical and operational team. You'll work with our dispatch teams and other healthcare professionals to make sure patients receive timely, joined-up care, communicating clearly and completing accurate clinical handovers. → Maintaining excellent clinical standards You'll work within IC24's clinical governance framework and in accordance with the requirements of your professional registration. You'll maintain accurate clinical records, contribute to audit and quality improvement and keep your clinical knowledge and skills up to date through CPD, supervision and reflective practice. You'll also support colleagues and contribute to clinical supervision, coaching and development where appropriate. This is advanced practice beyond the consulting room No two home visits will necessarily look the same. You'll see patients with everything from relatively straightforward acute illness to complex physical, mental health and social needs. Sometimes the answer will be clear. Sometimes you'll need to piece together limited information, understand the patient's home circumstances and make an important clinical decision without the immediate support of a full clinical team around you.
You'll probably thrive here if you: Enjoy autonomous clinical practice Are confident assessing undifferentiated patients in their own homes Have a high level of clinical reasoning and decision-making ability Are comfortable managing risk and uncertainty Can make safe decisions without having every investigation immediately available Are confident caring for frail, complex and end-of-life patients Communicate clearly and compassionately with patients, relatives and carers Can adapt your communication in difficult or sensitive situations Are comfortable working independently while knowing when to seek support If you prefer working in a predictable clinic environment with immediate access to a wider clinical team, home visiting may not be the right fit. But if you value clinical autonomy, variety and the opportunity to bring advanced care directly to patients in their own homes, we'd like to hear from you.
You'll: Hold current professional registration with the NMC, HCPC or GPhC, with at least five years' post-qualification and registration experience Hold a Masters Degree / Level 7 qualification Advanced Clinical Practice or will be working towards one Hold an Enhanced Assessment and Consultation qualification at Level 7 Hold a Minor Illness qualification at 7 Be a registered Independent Prescriber Have advanced assessment, examination, diagnosis and treatment skills across an agreed scope of practice, including newborns, paediatrics and end-of-life care Have significant post-registration experience including autonomous practice at an advanced level Be confident working independently in a patient's home Have strong clinical reasoning and risk-assessment skills Be able to work flexibly, including unsociable hours, weekends and Bank Holidays Hold a full UK driving license and will be comfortable driving both manual and automatic vehicles Who are we? Package Description: Urgent Primary Care Doctor – Bank (Urgent Treatment Centres) Be the clinician patients need when their care can't wait. Urgent care is General Practice at its most immediate. Patients don't always know what they need when they reach us. They may have been referred through NHS 111, redirected from an Emergency Department or walked into an Urgent Treatment Centre looking for help. What they need is an experienced doctor who can listen, assess, make confident clinical decisions and get them to the right care. At IC24, we're looking for Urgent Primary Care Doctors to work across our Urgent Treatment Centres in East Kent, including William Harvey Hospital in Ashford, Kent and Canterbury Hospital in Canterbury and Queen Elizabeth The Queen Mother Hospital in Margate. You'll work as part of a multidisciplinary urgent care team, providing safe, timely and effective care while helping patients access the most appropriate treatment for their needs. If you enjoy the variety of General Practice, the pace of urgent care and the clinical challenge of never quite knowing what is coming through the door next, this could be the role for you. What you'll focus on → Assessing and treating patients who need urgent care You'll provide face-to-face assessment, diagnosis and treatment for patients presenting with a broad range of acute illness, minor illness and injury. Patients may come to you through NHS 111, as walk-ins or following redirection from an Emergency Department. You'll assess what's happening, decide what needs to happen next and ensure patients receive appropriate treatment, referral and safety-netting. → Making confident clinical decisions Urgent Treatment Centres require sound clinical judgement. You'll often be working with patients you haven't met before, sometimes with limited background information and sometimes when they're anxious, unwell or experiencing difficult circumstances. You'll need to be able to assess risk, manage uncertainty and make safe decisions under pressure, recognising when a patient can be treated and discharged and when they need escalation or an alternative care setting. → Working at the heart of urgent care You'll work in a fast-moving clinical environment where patient needs can vary considerably from one consultation to the next. You'll see a broad mix of acute illness, minor illness and injury, using your General Practice and urgent care experience to determine the most appropriate course of action. Some patients will need assessment, treatment and discharge. Others may need further investigation, onward referral or escalation into another part of the healthcare system. Being able to recognise that difference – and make the right decision safely – is a fundamental part of the role. → Working as one multidisciplinary team Good urgent care depends on people working well together. You'll work alongside nurses, advanced practitioners, reception colleagues, other doctors and allied healthcare professionals, as well as colleagues across Emergency Departments, NHS 111, community services and primary care. Within our East Kent Urgent Treatment Centres, you'll work as part of the wider East Kent UTC Alliance, alongside colleagues from EKHUFT, Invicta Health CIC, Thanet Health CIC and IC24. You'll communicate clearly, share clinical expertise and work collaboratively to make sure patients move smoothly through the system. → Providing the right care in the right place Not every patient who arrives at hospital needs Emergency Department care. You'll help assess, prioritise and redirect patients where clinically appropriate, supporting people to access the right level of care while helping reduce unnecessary pressure elsewhere in the system. Whether you're treating and discharging a patient, referring them to another service or escalating their care, the patient and their safety will be at the centre of your decision-making. → Maintaining excellent clinical standards You'll work within IC24's clinical governance framework and prescribing formulary, maintaining accurate and timely clinical records and working in accordance with GMC guidance and relevant national and local clinical standards. You'll maintain your CPD and essential learning and have opportunities to contribute to audit, learning events, education and quality improvement. Some sessions will be busy. Priorities can change quickly. You'll sometimes need to make important clinical decisions with limited information and balance the needs of several patients at once.
You'll probably thrive here if you: Enjoy the variety and unpredictability of urgent care Are confident assessing undifferentiated patients Can make sound clinical decisions under pressure Are comfortable managing risk and uncertainty Have strong face-to-face consultation and triage skills Communicate clearly and compassionately with patients, relatives and carers Enjoy working within a busy multidisciplinary clinical environment Can adapt when service demands change unexpectedly Are comfortable working independently while knowing when to seek support If you prefer a predictable caseload and a slower-paced clinical environment, an Urgent Treatment Centre may not be the right fit. But if you enjoy thinking on your feet, value clinical variety and get satisfaction from helping patients at the point they need care most, we'd like to hear from you.
What you'll bring You'll: Hold current GMC registration Be an MRCGP Graduate Have relevant clinical experience within areas such as Primary Care, Emergency Medicine, A&E or previous Out of Hours experience Have experience of face-to-face clinical consultation Be confident assessing and managing undifferentiated patients Be comfortable making autonomous clinical decisions and managing risk Have strong communication and clinical record-keeping skills Be able to work effectively as part of a multidisciplinary team The environment you'll work in Urgent care operates around the needs of our patients, so flexibility is an important part of this role. Hours include evenings, weekends, overnights and Bank Holidays. You'll work within our East Kent Urgent Treatment Centres, alongside colleagues from across the wider urgent and emergency care system. It's a responsive clinical environment where demand and priorities can change quickly. You'll need to be comfortable adapting while maintaining safe, effective and patient-centred care.
